Respected Sir/Madam,

We are conducting a survey to understand the availability of paediatric refraction and spectacle dispensing services in independent optical and optometry practices in India. 

We request you to participate in this survey if you fit in any of the following criteria:
- *an owner of an independent optical or an independent optometry practice in India* 
- *person in charge of a branch of an independent optical or an independent optometry practice in India* 
- *If there are multiple outlets of your practice, the form may be filled separately for each practice by respective store in charge* 

 _Note: All information shared in the survey will be kept strictly confidential, and no personal or practice details will be disclosed._ _Only aggregated information will be published in any reports or publications resulting from this study.
